Malnutrition in Hospitalized Patients: Determining the Prevelence of Vitamin D Deficiency

This study examined the prevalence of vitamin D deficiency among hospitalized adults at Grady Memorial Hospital using electronic medical record data from January 2013 to October 2024. Vitamin D status was assessed by serum 25-hydroxyvitamin D levels. Among 29,037 vitamin D tests ordered during this period, 23,111 results (79.6%) were abnormal.<br /><br />A pilot analysis from January 1 to 17, 2024 included 286 patients tested for vitamin D. Of these, 205 patients (71.6%) had abnormal results, and all abnormal results were low, meaning deficient. The lowest measured level was 7 ng/mL, and 42 samples (20.5%) showed severe deficiency with levels below 10 ng/mL.<br /><br />Patients with low vitamin D had a median age of 65 years. Women accounted for 59% of abnormal low results, while men made up 40.5%. Most patients with deficiency were Black or African American (73.6%), followed by White or Caucasian patients (18%), suggesting possible racial disparities. The data also showed that while 25.3% of patients had a normal BMI, 17.5% were obese, indicating a potential relationship between higher BMI and vitamin D deficiency.<br /><br />Overall, the study found a very high prevalence of vitamin D deficiency in hospitalized adults, with nearly three out of four tested samples below the minimum cutoff and many showing severe deficiency. The authors conclude that vitamin D deficiency is a major concern in this hospitalized population and recommend further investigation into risk factors, vulnerable groups, and clinical consequences to improve screening and management.
