This pilot study evaluated whether pre-discharge lung and IVC ultrasound could predict short-term outcomes in patients hospitalized with acute decompensated heart failure (ADHF). Pulmonary B-lines, which indicate pulmonary edema, and IVC diameter/collapsibility were measured in 37 ADHF patients before discharge. Patients were followed for readmission or death at 30 and 90 days using telephone follow-up and chart review.<br /><br />The composite outcome of readmission or death occurred in 18.9% of patients at 30 days and 29.7% at 90 days. At 90 days, patients with 4 B-lines had significantly worse outcomes, and those who experienced readmission or death had higher overall B-line counts than those who did not. However, these differences were not significant at 30 days. IVC abnormalities showed a similar trend: dilated, non-collapsible IVCs were more common in patients with adverse 90-day outcomes, but this was not statistically significant.<br /><br />Overall, the study found encouraging trends suggesting that higher pre-discharge B-line counts may help identify ADHF patients at greater risk for readmission or death within 90 days, while IVC measures did not clearly predict outcomes in this small sample. The authors concluded that the study was likely underpowered and that larger studies are needed. They proposed expanding this approach across heart failure patients at the Miami VA, along with training hospitalists in pre-discharge ultrasound assessment, to improve care quality and reduce costs.
