Longitudinal Humoral Response to Sars-Cov-2 Infection and/or Immunization Among Healthcare-Workers

This prospective study examined the longitudinal SARS-CoV-2 humoral immune response in 80 healthcare workers at a community safety-net hospital in the South Bronx over four phases from May 2020 to May/June 2022: pre-vaccination, post-vaccination, and post-booster. Antibody response was measured using anti-spike AUC by ELISA.<br /><br />Overall, participants who had prior SARS-CoV-2 infection showed higher antibody titers than those without prior infection during the pre-vaccination period. However, antibody levels tended to decline over time before vaccination, especially among previously infected individuals, while uninfected participants showed little change. After Pfizer mRNA vaccination, antibody titers increased significantly in all participants, and those with prior infection still had higher levels than those without prior infection.<br /><br />The strongest post-vaccination response was seen in Phase 3. After booster vaccination, antibody levels depended on time since booster. When time from booster was controlled for, participants without prior infection showed a significant rise in antibody titers after the booster, whereas those with prior infection did not show a significant further increase. This suggests that booster benefit may be more pronounced in infection-naïve individuals.<br /><br />No significant associations were found between antibody levels and age or sex. Ethnicity appeared to influence response, with Asian participants showing higher baseline antibody responses, particularly if they had prior infection.<br /><br />The authors conclude that prior infection and vaccination together produce a strong humoral response, but booster doses may lead to greater relative increases in those without previous infection. They suggest that pre-existing immune memory may affect the magnitude of response and that further studies are needed to understand the impact of repeated boosters.
