Long-Term Hospital Readmission Rates Based on Teaching vs. Non-Teaching Status

This study examined whether long-term readmission rates for pneumonia, heart failure (HF), and acute myocardial infarction (AMI) differed between teaching and non-teaching U.S. hospitals from 2005–08 through 2017–19. Using CMS Hospital Compare data, hospitals were classified as non-teaching, teaching non-COTH, or teaching COTH (Council of Teaching Hospital and Health Systems). Trends were analyzed with mixed linear models adjusted for hospital and community characteristics.<br /><br />At baseline, COTH teaching hospitals had slightly higher readmission rates than non-teaching hospitals for pneumonia and AMI, while HF rates were similar. Non-COTH teaching hospitals were generally similar to non-teaching hospitals, though they had lower HF readmissions in some adjusted analyses.<br /><br />Over time, readmission rates declined across all hospital types for pneumonia, HF, and AMI. However, the decline was larger in COTH teaching hospitals. By 2017–19, COTH hospitals showed significantly greater reductions compared with non-teaching hospitals for pneumonia and AMI. In contrast, non-COTH teaching hospitals showed a small but significant increase in HF readmissions relative to non-teaching hospitals.<br /><br />Overall, the results suggest that although COTH teaching hospitals started with somewhat higher readmission rates, they experienced the greatest improvement over the study period. This indicates that teaching hospitals, especially major academic centers, may have adapted more effectively to efforts to reduce readmissions.
