Lipoprotein (A) and Cardiovascular Risk: Racial and Gender Disparities and the Impact on Coronary Interventions

This study examined lipoprotein(a) [Lp(a)], a genetically inherited biomarker associated with coronary artery disease (CAD), in 833 adults tested at a community hospital from January 2022 to December 2024. Patients were grouped as having high Lp(a) (≥125 nmol/L) or normal Lp(a). Researchers assessed differences by race and sex, and analyzed links between elevated Lp(a), CAD, and coronary interventions using chi-square testing.<br /><br />Key findings showed that 27% of patients had elevated Lp(a). High Lp(a) was significantly more common in African American patients than in non–African American patients (43.2% vs 21.4%). Among African American patients, women had higher Lp(a) levels than men (49.1% vs 37.0%). Elevated Lp(a) was also associated with a higher prevalence of CAD (68.6% vs 54.9%). In patients with CAD, those with elevated Lp(a) were more likely to have undergone PCI or CABG (37.9% vs 23.5%).<br /><br />The study suggests that elevated Lp(a) is especially prevalent among African American women and is strongly associated with worse cardiovascular outcomes and greater need for coronary revascularization. The authors conclude that routine Lp(a) testing could improve prevention, support earlier cardiology referral, and help promote more equitable, population-specific cardiovascular risk assessment.
