This project evaluated two EHR-based Best Practice Advisories (BPAs) designed to improve inpatient workflow, patient safety, and compliance with CMS requirements.<br /><br />The first intervention targeted medication reconciliation at discharge. BPAs alerted providers when reconciliation was incomplete, offered a direct link to complete it, and enabled “on-the-fly” reconciliation for new orders. A hard stop prevented nursing from printing the after-visit summary until reconciliation was finished. This increased discharge medication reconciliation compliance from 83.6% to 97.1%, improving the organization’s EPIC benchmark ranking from the 12th to the 77th percentile. Providers reported that the alerts were helpful and not overly burdensome.<br /><br />The second intervention addressed missing admission orders. The team worked with stakeholders to create a BPA that prompted clinicians to enter the admission order before any other documentation. The alert was reviewed by the Physician Practice Council and adjusted to allow urgent orders. Provider education and ongoing monitoring supported implementation. This BPA resulted in a 90.7% immediate reduction in delayed admission orders, improving adherence to CMS standards. Providers viewed the alert as non-intrusive and responded positively.<br /><br />Overall, the study shows that thoughtfully designed EHR interventions can substantially improve compliance, streamline workflows, and enhance patient safety. The authors note that future work will examine whether these changes also reduce medication errors and adverse events after discharge.
