Leveling Up: An Iterative, Online Approach to Peer Feedback

This project addressed a common challenge in clinician development: peer feedback is valuable, but staff often complete too few peer evaluations because the process is awkward, time-consuming, or hard to access. At Yale New Haven Hospital, the original evaluation system produced only about 30–40 peer reviews per year across a team of 180 clinicians.<br /><br />To improve participation, the team used an iterative PDSA approach to redesign the process. First, they moved from a more cumbersome E-Value platform to a mobile-friendly Qualtrics survey with no separate login. Later, they shortened the evaluation to focus on qualitative, actionable feedback and began sending direct email requests asking clinicians to evaluate a specific peer.<br /><br />The results were dramatic. Annual completion rates stayed around 30 for the first two cycles, but after the 2022 redesign, 372 evaluations were completed between February and October alone.<br /><br />A follow-up survey of 55 users showed which changes mattered most. The most commonly cited improvements were:<br />- shorter evaluation form (65%)<br />- no separate login required (61%)<br />- mobile accessibility (58%)<br />- direct email request from leadership (48%)<br />- request about a specific peer (45%)<br />- open access throughout the year (28%)<br /><br />When asked for the single most important change, 37% chose the shorter form and 30% chose the direct email request.<br /><br />Overall, the study found that a short, accessible, email-driven peer evaluation tool substantially increased feedback volume. Respondents viewed the changes positively, especially the simplified form and direct solicitation. The authors conclude that this approach improves peer evaluation completion, though further study is needed to determine how useful the feedback is and whether it changes clinician practice.
