Let's Role Play: Utilization of Sim Lab for App Readiness Aimed at Independent Practice

This project addressed the challenge of preparing newly graduated advanced practice providers (APPs) for independent practice in hospital medicine. Because APP training varies widely and is much shorter than physician residency training, the program created a structured simulation-based orientation to improve readiness, confidence, and clinical decision-making.<br /><br />Newly hired APPs participated in a dedicated simulation lab led by experienced APPs using seven core scenarios: Goals of Care, Chest Pain, Respiratory Failure, Diabetes, Sepsis, Stroke, and Pancreatitis. Simulations used either high-fidelity mannequins or standardized patient actors. During each session, participants performed a history and physical exam, interpreted clinical data, and developed management plans. Each simulation ended with self-assessment and group debriefing, followed by targeted feedback from APP leaders.<br /><br />Survey results from 25 evaluations showed strong positive responses. In 76% of surveys, participants strongly agreed that the simulations were clinically relevant and that facilitators were knowledgeable and effective. In 80% of surveys, participants reported learning new information, and 68% said they intended to apply what they learned in practice. Feedback comments suggested improved communication skills, better recognition of patient decompensation, and appreciation for more detailed feedback and patient-actor input.<br /><br />Overall, the initial findings suggest that simulation is a valuable tool for improving perceived preparedness among new APPs. Ongoing follow-up will assess whether these gains translate into clinical practice and will help identify areas for improvement and additional training topics.
