This case report describes a 59-year-old Hispanic man with alcoholic cirrhosis and esophageal varices who presented with abdominal pain and worsening renal function. On admission, he had leukocytosis, elevated creatinine, mild hyperbilirubinemia, and elevated liver enzymes. He was initially treated for suspected spontaneous bacterial peritonitis with ceftriaxone, but later developed hypotension requiring ICU care. Because of hematuria and low complement levels, a renal biopsy was performed.<br /><br />Biopsy showed two concurrent kidney diseases: IgA nephropathy and LECT-2 (leukocyte chemotactic factor 2) amyloidosis, along with moderate interstitial fibrosis and acute tubular injury. His course was complicated by worsening acute kidney injury, a left perinephric hematoma after biopsy, and a left popliteal vein thrombosis requiring IVC filter placement.<br /><br />The report highlights that amyloidosis results from abnormal deposition of insoluble protein fibrils in organs and can cause progressive dysfunction. LECT-2 amyloidosis is a recently recognized form that can affect the kidney and liver, and it may be especially common among Hispanic patients in the United States. It is often diagnosed by kidney or liver biopsy and has been reported to account for a significant portion of hepatic amyloid cases.<br /><br />The authors emphasize that LECT-2 renal amyloidosis may coexist with glomerular diseases such as IgA nephropathy, particularly in patients with chronic inflammatory conditions like chronic liver disease. Clinicians should consider LECT-2 amyloidosis as a potential cause of renal and hepatic disease, especially in Hispanic populations.
