Learning Environment on Rounds: Peer Observation Study of Pediatric Hospitalists

This prospective observational study examined how pediatric hospitalists create a professional learning environment during family-centered rounds and whether technique use varies by years of experience. Conducted at a tertiary pediatric hospital from July 2022 to July 2023, the study used peer observation and a 19-item learning environment form spanning six categories: learning climate, control of session, education, evaluation, feedback, and self-directed learning.<br /><br />All pediatric hospitalists in the division were eligible. After an introductory session, participants were observed during three rounds sessions, each at a different experience tier: 1–6 years, 6–11 years, and more than 11 years post-training. Observers recorded the use of techniques and provided direct feedback afterward. Of 48 eligible participants, 31 completed the program. There were 93 observations, with 80 complete forms included in analysis.<br /><br />Results showed that pediatric hospitalists frequently used many positive teaching behaviors. Six techniques were observed 100% of the time, all within the learning climate category, including making learners feel comfortable, allowing uninterrupted presentations, expressing respect, treating learners kindly, modeling caring behavior, and showing enthusiasm. Nine additional techniques were used in 80–98.8% of observations. Only four techniques were used less often, offering opportunities for improvement. Experience had little effect on technique use; the only statistically significant difference across tiers was asking learners to highlight key physical exam findings.<br /><br />Overall, the study suggests that pediatric hospitalists commonly use many effective strategies to support learning on rounds, regardless of experience level. The authors recommend further education to increase use of the least observed techniques and future research linking observer findings with learner perceptions of the learning environment.
