Learner Perceptions of Teaching Quantity and Techniques Utilized During Pediatric Bedside Rounds

This prospective cohort study examined how pediatric residents perceive bedside teaching during family-centered rounds compared with observing trainees. Conducted at Phoenix Children’s Hospital from April to August 2025, the study involved 20 observed rounds led by 16 pediatric hospitalists. In total, 39 observation forms were completed, and 44 of 59 post-round surveys were returned by residents, mostly PGY-1s.<br /><br />Before the study, residents reviewed a video describing 11 teaching methods. During rounds, observers recorded the number and types of teaching techniques used, and participating residents later reported what they perceived.<br /><br />The main finding was that teaching quantity was perceived similarly by both groups. Observers identified an average of 9.7 teaching points per rounds session. Residents reported 0–5 teaching points in 25% of cases, 6–10 in 50%, and 11 in 25%. This suggests residents did notice bedside teaching, challenging the idea that such teaching is often overlooked in busy clinical settings.<br /><br />However, perceptions of teaching techniques differed between groups. Residents most often identified broadening, up-the-ladder, and multi-answer/no right answer techniques. Observers most often identified targeting, teaching the team through teaching the family, and multi-answer/no right answer. These differences imply that observers and participants may pay attention to different aspects of the same teaching interaction.<br /><br />The authors conclude that faculty development should emphasize intentional use of bedside teaching strategies and clearer recognition of those strategies by learners. This could help ensure teaching moments are both delivered effectively and recognized during bedside rounds.
