Lean Rounds Initiative to Optimize Inpatient Teaching Rounds

The project evaluated a “Lean Rounds” initiative designed to improve efficiency in inpatient teaching rounds while maintaining patient care and education quality. The goal was for 70% of resident teaching teams in general medicine and cardiology to finish rounds by 11:00am.<br /><br />During the development phase, teams that used Lean Rounds practices more consistently reported higher satisfaction with patient care, communication, efficiency, and ending time. They reported slightly lower satisfaction with educational content than low users, but satisfaction still remained above 80%. High users also finished rounds earlier than low users, though only 29% met the 11:00am target.<br /><br />In the implementation phase, residents whose rounds ended by 11:00am or 11:30am reported much higher satisfaction with both patient care and education components.<br /><br />The initiative was developed through an interdisciplinary workgroup of hospitalists, cardiologists, residents, and residency leadership. It included guidelines and educational materials focused on strategies such as focused presentations, enhanced pre-rounding, involving the bedside nurse, minimizing repetition, strategic card flipping, bedside rounds when possible, and quick transitions between rooms.<br /><br />Overall, the findings suggest that adopting Lean Rounds practices can improve rounding efficiency without reducing the quality of patient care or educational value. The team plans to intensify efforts, investigate barriers to meeting the 11:00am goal, continue monitoring satisfaction, and formally incorporate Lean Rounds training into resident orientation and attending onboarding.
