Lean In, Level Up: Dusting Off Interdisciplinary Rounds After Covid-19

UC San Diego Health used a two-day Lean-based rapid process improvement workshop (RPIW) to redesign its Focused Interdisciplinary Team (FIT) bedside rounding model after COVID-19 disrupted and diluted the original process. FIT rounding had previously improved patient flow, communication, and teamwork across seven units, but pandemic-related changes shifted rounding away from the bedside.<br /><br />The workshop brought together representatives from hospital medicine, nursing, care management, social work, pharmacy, rehab services, patient experience, information technology, and health systems operations, including an actual patient. On day one, participants learned Lean concepts, mapped the current workflow, identified root causes of problems, and defined goals and guiding principles for the future state. On day two, workgroups refined standard work, logistics, EHR documentation, and patient-facing materials, while also creating implementation, communication, and measurement plans.<br /><br />The team’s redesign aimed to improve patient-centeredness, strengthen interdisciplinary communication, and optimize patient flow and care quality metrics. Participants reported that the process was highly collaborative and educational, increasing appreciation for other disciplines. Key lessons included the value of Lean training, patient involvement to preserve patient-centeredness, and IT participation to accelerate EHR tool development.<br /><br />Overall, the project shows that a multidisciplinary, grassroots Lean RPIW is an effective way to redesign interdisciplinary rounding and build buy-in for sustainable change.
