Transthoracic echocardiography (TTE) is emphasized as the first-line diagnostic test, with contrast echocardiography, MRI, and genetic testing serving as additional tools when needed.<br /><br />Clinical presentation is highly variable. The most frequent manifestation is heart failure, but arrhythmias and thromboembolic complications are also common. Atrial fibrillation is a notable arrhythmic presentation. The article also notes that LVNC can be classified into several clinical types, including isolated, arrhythmogenic, dilated, hypertrophic, restrictive, biventricular, and forms associated with congenital heart disease.<br /><br />The authors stress that diagnosis in adults is often delayed because LVNC can resemble more common conditions such as ischemic heart disease, hypertension, and other cardiomyopathies. Early recognition is important because timely treatment can improve prognosis and reduce serious complications such as heart failure, thromboembolism, and ventricular arrhythmias.<br /><br />Management may include heart failure therapy, oral anticoagulation, antiarrhythmic medications, cardiac resynchronization therapy, implantable cardioverter-defibrillator placement, or heart transplantation in advanced cases.
