Interventional Hospital Medicine - When Hospitalists Become Proceduralists!

Baylor University Medical Center described the creation and growth of a hospitalist-run procedure service launched in 2018 to perform bedside procedures such as paracentesis, thoracentesis, lumbar punctures, arthrocentesis, central lines, temporary dialysis access, complex IV access, skin biopsies, and suprapubic catheter exchanges. The team expanded from 2 to 16 proceduralists over four years and now performs about 2,000 procedures annually.<br /><br />The program was started to address a common problem: many routine bedside procedures had shifted to interventional radiology (IR), causing delays in care, longer hospital stays, increased costs, and heavy IR workload. By moving simpler procedures to trained hospitalists at the bedside using ultrasound guidance, the team reduced IR wait times by 20% in the first month.<br /><br />The presentation highlighted several benefits of the service: improved patient satisfaction, decreased length of stay, reduced transportation and procedural costs, increased hospitalist and facility revenue, and better efficiency in care delivery. It also emphasized the educational value of the program, noting a popular medical student and residency elective designed to train future physicians in procedural skills.<br /><br />Over four years, the team completed more than 8,000 bedside procedures with a complication rate comparable to or better than interventional radiology. The authors concluded that a well-trained hospitalist procedure team can significantly enhance the value of hospital medicine while improving patient flow, reducing delays, and preserving important procedural skills in internal medicine training.
