Institutional Point of Care Ultrasound Training for Hospitalists

This project addressed a common barrier to point-of-care ultrasound (POCUS) use in hospital medicine: lack of training. The hospital medicine group created a formal, workshop-based POCUS training program to help hospitalists use ultrasound more routinely in clinical care.<br /><br />The goal was to design, implement, and evaluate a comprehensive training program. Between May and October 2024, 26 providers attended two workshops, and 19 completed pre- and post-workshop surveys (73% response rate).<br /><br />Training took place in the simulation center and focused on cardiac, pulmonary, abdominal, and vascular ultrasound. The workshops used standardized patients instead of models. Participants rotated through three rooms, with one standardized patient and one ultrasound machine per room. Pre- and post-tests measured both knowledge and comfort with POCUS.<br /><br />Results showed improvement across all measured areas. The average ability to correctly identify vascular congestion increased from 50% to 100%, and identification of a lung point improved from 27% to 60%. Provider comfort also rose substantially: comfort using POCUS increased from 10% to 77%, comfort using POCUS to evaluate a decompensating patient from 37% to 72%, comfort assessing ejection fraction from 10% to 68%, and comfort with image optimization from 5% to 78%.<br /><br />The authors conclude that POCUS training improved provider knowledge and confidence. They also developed a broader multimodal program that includes didactic teaching, simulation-based workshops, and monthly electronic teaching points. Next steps include tracking POCUS documentation to determine whether training leads to increased real-time ultrasound use in clinical practice.
