Inpatient Teaching Attending Experiences Across an Academic Learning Health System

This survey study examined inpatient attending physicians who supervised General Medicine teaching services across two Atrium Health academic sites: Atrium Health Wake Forest Baptist and Atrium Health Carolinas Medical Center. The goal was to understand how attending physicians’ work structure, teaching environment, and personal circumstances relate to their ability to teach learners and to symptoms of burnout.<br /><br />A 29-question anonymous survey was sent to 54 attendings during the 2022–2023 academic year, with a 55%–56% response rate. Results showed that attendings working a 50% inpatient/50% outpatient schedule said the number of learners on the team and the admissions model affected how well they could teach. Attendings with protected administrative time also reported that non-patient-care duties interfered with teaching.<br /><br />Burnout symptoms were common. Emotional exhaustion was reported by 80% of respondents, depersonalization by 37%, and reduced personal achievement by 57% at least “sometimes” or “often.” Greater emotional exhaustion was associated with having 10 weeks of inpatient teaching service and with 10 years at the current institution. Depersonalization was also linked to 10 weeks of inpatient service and being the primary caregiver for a family member or friend. No significant differences in burnout were found between the two health system sites.<br /><br />The authors conclude that “one size does not fit all” when addressing attending physician wellbeing. They recommend focusing on individual and role-specific factors that may contribute to burnout and reduced professional fulfillment, and suggest follow-up focus groups or surveys to better identify contributors such as non-clinical demands and feeling undervalued.
