This project aimed to improve naloxone kit distribution to at-risk patients. In March 2024, naloxone kit orders reached 59% of the expected monthly target, exceeding the initial goal of 20%. Provider SmartPhrase use also increased, suggesting better bedside education and confirmation that patients receiving kits were appropriately counseled.<br /><br />Documentation improved as well. Nursing education sessions helped increase the number of kits correctly marked as dispensed in the medication administration record (MAR). The team also noted that some orders labeled as “lost” may actually have been dispensed but not documented properly in the MAR, which may have affected measured performance.<br /><br />Despite these gains, long-term sustainability remains a challenge. Current workflows depend heavily on communication between providers and nursing staff, which can be difficult during busy discharge periods. Secure Epic chat reminders are time-consuming, and ongoing education is harder with a large residency group.<br /><br />To support continued improvement, the team proposed several next steps:<br />- Create a Best Practice Advisory in the electronic medical record to prompt naloxone dispensing for at-risk patients.<br />- Add education on opioid use disorder and naloxone distribution to the resident curriculum.<br />- Include naloxone eligibility discussions in daily interdisciplinary rounds.<br /><br />Overall, the interventions were successful in improving naloxone ordering, provider education, and documentation, but additional system-level changes are needed to make the process more reliable and sustainable.
