Inhaled Bronchodilators: Novel Approach to Improved Outcomes in the Treatment of Pneumonia Requiring Hospitalization in Absence of Underlying Lung Disease?

This retrospective multicenter cohort study examined whether inhaled bronchodilators improve outcomes in hospitalized patients with pneumonia who do not have known underlying lung disease (such as asthma or COPD). The study used data from 152 HCA Healthcare hospitals across 20 states and included patients hospitalized between January 1 and December 31, 2019.<br /><br />A total of 1,593 patients were analyzed, and 32.89% received an inhaled bronchodilator at least once within 24 hours of admission. The primary outcomes were hospital length of stay, ICU admission, and in-hospital mortality.<br /><br />Results showed that patients who received inhaled bronchodilators had a significantly shorter hospital stay, averaging 3.66 days less than those who did not receive them. They also had a lower rate of ICU admission. Patients who did not receive bronchodilators were more likely to require ICU care (OR 1.279, 95% CI 1.032–1.585; p=0.0244). However, there was no statistically significant difference in in-hospital mortality between the two groups.<br /><br />Overall, the authors concluded that inhaled bronchodilator use may be associated with improved clinical outcomes in hospitalized pneumonia patients without underlying lung disease, particularly by reducing length of stay and ICU admission. They suggest that bronchodilators may have a beneficial role in this population, though further research would be needed to confirm causation and guide routine use.
