This quality improvement project aimed to increase prescribing of SGLT2 inhibitors for hospitalized patients with congestive heart failure (CHF) on Medical Teaching Service teams at Trinity Health Ann Arbor & Livingston. Because SGLT2 inhibitors are part of guideline-directed medical therapy and have been shown to reduce cardiovascular death and heart failure hospitalizations by about 30%, the team set a SMART goal of raising prescribing rates to 55% by December 2024.<br /><br />The project used a retrospective chart review based on EMR-generated reports from December 2023 to April 2024. All CHF admissions to MTS teams were included. The first intervention cycle (PDSA 1) focused on monthly teaching sessions, educational info sheets, and a dotphrase (.aahfdoc) to simplify documentation and prescribing workflow. Data were reviewed every two weeks and manually filtered to identify only MTS patients.<br /><br />In the second cycle (PDSA 2), the team shifted to more informal “on the fly” teaching, gathered resident feedback on the dotphrase, emphasized its use more directly, and continued formal teaching sessions. Challenges included inconsistent coding, difficulty ensuring all residents were educated, and the inability to track dotphrase usage.<br /><br />Overall, prescribing rates improved for CHF patients on MTS teams and exceeded the 55% target. The largest gains occurred during PDSA 2, suggesting that sustained engagement and resident buy-in were key to success. The team plans to expand group education, improve access to the dotphrase among residents and attendings, and further embed this practice into residency culture.
