Incidental Bony Metastasis Resulting in Diagnosis of Male Breast Cancer

This case report describes a 62-year-old man who presented with fatigue, weakness, and ankle pain after a recent COVID-19 infection. Initial workup showed bilateral acute deep vein thromboses, and CT pulmonary angiography unexpectedly revealed multiple lucent bone lesions in the spine, scapulae, and ribs, raising concern for metastatic disease. Further testing for common malignancies and multiple myeloma was unrevealing, and a right iliac bone biopsy showed metastatic carcinoma of unknown origin.<br /><br />At follow-up, a careful physical exam identified a small scab over the left nipple with an underlying palpable mass. Diagnostic mammography demonstrated an irregular mass suspicious for malignancy, and biopsy confirmed invasive ductal carcinoma of the breast as the primary source of the bone metastases.<br /><br />The report emphasizes that male breast cancer is rare, accounting for less than 1% of all breast cancers and less than 1% of cancers in men. Because men are not routinely screened, diagnosis is often delayed and disease is frequently more advanced at presentation. Risk factors include family history, older age, obesity, cirrhosis, radiation exposure, and BRCA2 mutation.<br /><br />The authors highlight that breast cancer should remain on the differential diagnosis for men with a palpable breast mass or metastatic disease of unknown origin. Increased awareness among both patients and clinicians may lead to earlier recognition, diagnosis, and treatment.
