Incidence of Acute Ischemic Stroke and Tia in Hospitalized Covid19-Positive Patients Compared to Influenza-Positive Patients

This retrospective study examined whether hospitalized patients with COVID-19 had a higher incidence of TIA or ischemic stroke than hospitalized patients with influenza in the HCA hospital network from January to June 2020. The authors used influenza as the comparison group because influenza itself is known to increase stroke risk, making it a more appropriate control than the general population.<br /><br />The study included 89,469 patients total: 25,156 with COVID-19 and 64,313 with influenza. Among COVID-19 patients, 139 (0.55%) had a stroke or TIA, compared with 129 (0.20%) among influenza patients. Statistical analysis showed a significant association between infection type and stroke/TIA occurrence (P < .0001). The odds ratio indicated that COVID-19 patients were 2.273 times more likely to experience TIA/stroke than influenza patients, with a 95% confidence interval of 1.541 to 3.352. Logistic regression adjusting for age, sex, and race produced the same result, suggesting the association was not explained by demographic differences. There was no statistically significant age difference between the groups.<br /><br />The study concludes that TIA and ischemic stroke are serious complications of COVID-19, and that hospitalized COVID-19 patients appear to have more than double the risk of these events compared with hospitalized influenza patients.
