In-Hospital Outcomes and Sex Differences in Septic Heart Failure Patients

This study examined in-hospital outcomes and sex differences among septic patients with preexisting heart failure, comparing heart failure with preserved ejection fraction (HFpEF) and reduced ejection fraction (HFrEF). The HFrEF group had significantly worse outcomes than the HFpEF group, including higher mortality (14.1% vs 10.1%), cardiac arrest, cardiogenic shock, inotrope use, ventricular arrhythmias, acute kidney injury, mechanical circulatory support use, and major complications. HFrEF patients also generated higher total hospital charges ($141,960 vs $118,101). <br /><br />Some outcomes did not differ significantly, including respiratory failure, mechanical intubation, cerebrovascular accident, and length of stay. Pacemaker implantation was slightly lower in HFrEF, while PCI and mechanical circulatory support use were higher. <br /><br />Independent predictors of mortality in both groups included a prior history of stroke and liver disease. <br /><br />Sex-based analysis showed that male patients had worse outcomes in both the HFrEF and HFpEF subgroups. In contrast, female patients with HFrEF had higher rates of cerebrovascular accident. <br /><br />Overall, the findings suggest that sepsis in patients with HFrEF is associated with greater in-hospital morbidity, mortality, and cost than in HFpEF, and that male sex may confer additional risk.
