This study examined in-hospital mortality among adults admitted with a primary diagnosis of asthma using the 2019 US National Inpatient Sample. The authors identified 102,365 non-elective adult asthma admissions and evaluated associations between mortality and complications such as intubation, noninvasive positive-pressure ventilation (NPPV), pneumothorax, and shock using ICD-10 codes and multivariable regression.<br /><br />Overall in-hospital mortality was low at 0.4%. However, several factors were strongly associated with increased risk of death. Patients who required intubation for acute hypoxemic respiratory failure had much higher odds of mortality than those who were not intubated (adjusted OR 20.6, 95% CI 9.1–46.7, p<0.001). Shock was the strongest predictor of mortality (adjusted OR 76.7, 95% CI 23.6–249.1, p<0.001), and pneumothorax was also linked to higher mortality (adjusted OR 10.9, 95% CI 1.34–89.77, p=0.02). In contrast, NPPV was not significantly associated with mortality (adjusted OR 0.28, 95% CI 0.5–1.5, p=0.14).<br /><br />The study concludes that while most hospitalized asthma patients survive, severe respiratory failure requiring intubation and complications such as shock and pneumothorax mark a much higher-risk group. Early recognition and aggressive management of these high-risk features may help reduce mortality. NPPV did not appear to independently affect survival in this analysis.
