In-Hospital Mortality Among Those With Obstetric-Related Icu Admissions From 2016-2023

This study examined in-hospital mortality among patients with obstetric-related ICU admissions in the Vizient Clinical Data Base from January 2016 through June 2023. The cohort included people ages 12–55 admitted to cardiac, medical, surgical, or combined ICUs across 352 continuously reporting hospitals.<br /><br />Overall, about 2% of obstetric-related ICU patients died in the hospital. Mortality rose notably during the COVID-19 pandemic and increased again in early 2023. The most common principal diagnoses during mortality spikes were infections and viral diseases. Other frequent diagnoses included hypertensive disorders such as pre-eclampsia, infections/viral complications of pregnancy or childbirth, circulatory complications such as arrhythmia and cardiomyopathy, and puerperal sepsis.<br /><br />Patients who died in hospital commonly had serious comorbidities, including coagulopathy, heart failure, obesity, renal failure, cerebrovascular disease, and neurological disorders. In multivariable regression models, older age, Medicaid insurance, and residence in the Southern U.S. were associated with higher mortality risk. Significant clinical risk factors included metastatic cancer, cerebrovascular disease, renal failure, other neurological disorders, obesity, coagulopathy, and heart failure.<br /><br />Unexpectedly, Black race, paralysis, valvular disease, complicated hypertension, and depression were associated with lower odds of in-hospital mortality in the model.<br /><br />The authors conclude that rising mortality trends may be influenced by COVID-19 and related policy changes. They emphasize that broad, temporal analysis of obstetric ICU mortality can help identify high-risk patients and inform maternal health policy and clinical interventions, especially around comorbidities, prenatal care, and cesarean-related risk.
