Improving Provider Awareness of Patients' Inpatient Bed Status

This quality improvement project at the University of Oklahoma College of Medicine aimed to increase provider awareness of patients’ inpatient bed status, measured as “congruence” between the level of care physicians believed a patient had and the actual level of care. The target was to improve congruence to over 90% over six months.<br /><br />The team collected 1,350 data points from about 590 patients, including responses from 715 attendings and 635 residents. Baseline congruence was 78.2%, and after educational interventions it increased to 83.4%, a statistically significant improvement (p = 0.0367). By the end of the project, cumulative congruence reportedly exceeded 90%, meeting the study goal.<br /><br />Three interventions were used: a resident education session introducing a new EPIC feature showing inpatient level of care, email updates with baseline data and education, and a follow-up email with updated results and reminders. Data were collected weekly from February through June 2024 and analyzed with chi-square testing.<br /><br />Subgroup analyses showed improvement across providers, towers, and actual level of care categories. Attendings improved from 74.2% to 81.9%, while residents improved from 82.8% to 85.0%, eliminating the pre-intervention gap between them. South Tower improved from 70.6% to 84.1%, and North Tower from 64.8% to 77.7%, also removing the prior difference. Med/surg congruence improved from 80.5% to 90.4%; stepdown remained lower at 73.3% post-intervention.<br /><br />Overall, the study suggests that targeted education and communication can improve provider awareness of inpatient bed status and may help standardize understanding of stepdown utilization.
