Improving Medical Student Ecg Interpretation Skills With Spaced Repetition Quizzes

This study evaluated a spaced learning ECG curriculum designed to improve medical students’ ECG interpretation skills during clerkship. All students completed a 17-item ECG multiple-choice pretest and posttest at the start and end of the clerkship year. The study compared three groups: an LIC intervention group, a block clerkship control group, and an LIC control group. Statistical analysis included one-way ANOVA, Games-Howell post hoc testing, and Hedges’ g to estimate effect size.<br /><br />Results showed that the LIC intervention group performed significantly better on the posttest than both control groups. The mean posttest score was 12.4 for the LIC intervention group compared with 8.6 for the block clerkship control group and 9.4 for the LIC control group. These differences were statistically significant (p = 0.001), with large effect sizes, especially versus the block clerkship group (Hedges’ g = 1.53) and a moderate-to-large effect versus the LIC control group (Hedges’ g = 0.90).<br /><br />Item-level analysis suggested that knowledge gains were best retained for arrhythmias, AV blocks, and conduction abnormalities. Overall, the findings indicate that spaced repetition quizzes helped students maintain ECG interpretation competence and reduced the effects of forgetting over time.<br /><br />The authors conclude that a spaced learning ECG curriculum can successfully improve ECG interpretation skills in medical education and support the use of repeated low-stakes testing as an effective instructional strategy.
