Important Factors for Scaling and Sustaining Hospital at Home Program

This study examined barriers and facilitators to scaling and sustaining a Hospital at Home (HaH) program using an early supported discharge model at Atrium Health Wake Forest Baptist, launched in December 2020. HaH allows selected hospitalized patients to be discharged early and continue inpatient-level care at home, with nurse navigators helping educate patients and support shared decision-making.<br /><br />The authors conducted a retrospective cohort study using online surveys of 104 HaH patients and 55 referring providers. Patients were mostly White, non-Hispanic, and covered by private insurance or Medicare, with variation in education, employment, and household size.<br /><br />Key findings showed that some patients had limited understanding of what to expect from HaH when they agreed to it, but overall satisfaction was high. Patients особенно valued the care team’s coordination and teamwork. The involvement of nurse navigators appeared to improve patient understanding, acceptance, and engagement by giving clear instructions and allowing participation in the decision to enroll.<br /><br />Referring providers reported few barriers to using the program and generally understood the types of conditions treated by HaH beyond COVID-19. They strongly agreed that HaH benefits both patients and the health system.<br /><br />The study concludes that nurse navigators are important for improving patient awareness and acceptance of HaH. It also emphasizes that clearly communicating referral criteria and the range of eligible conditions is critical to maintaining provider referrals and supporting long-term program sustainability.
