Implementing Rapid Response Simulation Training for Medical Sub-Interns

This document describes a pilot educational intervention for medical students on a sub-internship rotation. The authors note that students entering residency are often expected to manage acute patient deterioration and rapid response scenarios, yet many have little formal training or exposure during clerkships. To address this gap, they incorporated a structured Rapid Response simulation (SIM) workshop into the Direct Care Hospitalist Service sub-internship.<br /><br />The simulation included four acute care cases: hypotension, tachyarrhythmias, respiratory distress/hypoxia, and altered mental status. The sessions emphasized initial assessment, gathering additional history, stabilization, appropriate labs/imaging, and immediate management. Before and after the training, students completed surveys measuring their comfort with handling these emergency scenarios.<br /><br />Eight students participated. After simulation training, self-reported confidence improved across all areas. Specifically, 62.5% felt comfortable responding to emergencies, 75% felt comfortable developing a care plan, 62.5% were comfortable managing hypotension, 75% were comfortable managing altered mentation and tachycardia, and 87.5% were comfortable managing hypoxia. Overall, the intervention was associated with a marked increase in student comfort in managing acutely ill patients.<br /><br />The authors conclude that rapid response simulation can be a valuable addition to the sub-internship experience, helping prepare medical students for the responsibilities they will face during residency.
