Implementation of a Digital Education Platform in a Tertiary Care Children's Hospital

This quality improvement project evaluated implementation of a digital discharge education (DDE) platform at a 298-bed freestanding children’s hospital. The team aimed to increase patient engagement with DDE to 25% among Pediatric Hospital Medicine patients admitted with bronchiolitis, asthma, or seizures by May 2025.<br /><br />The project used a key driver diagram and tracked three measures: the outcome measure was the percentage of patients who engaged with DDE, the process measure was the percentage sent DDE, and the balancing measure was the percentage who did not receive DDE after it was sent. Interventions included weekly reminders to on-service faculty and integrating DDE into discharge summary templates.<br /><br />Results from 1,191 patients showed special cause variation in both outcome and process measures. Engagement increased from 0% to 5.3%, while the rate of DDE being sent rose from 0% to 36%. The balancing measure showed no major change; overall, 8.6% of patients who were sent DDE did not receive it.<br /><br />The findings suggest that providers can be prompted to send DDE consistently, but patient engagement remains low. Possible reasons include inaccurate contact information in the electronic health record. Next steps include adding nursing instructions to send DDE within diagnosis-specific discharge order sets, building clinical decision support to automate DDE delivery during admission, improving contact verification with the admissions team, and conducting qualitative studies to better understand family barriers and the educational value of DDE.<br /><br />Key limitations include reliance on provider action, dependence on WiFi/data or smartphones, and exclusion of non-English or non-Spanish speaking patients.
