Implementation of Ehr Documentation Tool for Bedside Interdisciplinary Rounds

This project describes the design and pilot implementation of a real-time EHR documentation tool, the “FIT Navigator,” used during bedside interdisciplinary rounds at UC San Diego Health.<br /><br />The goal was to make key information discussed during rounds immediately available in the electronic record while also supporting three broader aims: improving communication of care plans and discharge needs, monitoring fidelity to the Focused Interdisciplinary Team (FIT) rounding model, and assessing effects on patient flow and care quality.<br /><br />A multidisciplinary team from hospital medicine, nursing, IT, and transformational healthcare created a simple embedded navigator with click-box fields to document:<br />- whether FIT rounds occurred and who attended,<br />- invasive device de-escalation considerations,<br />- the medical reason for continued hospitalization,<br />- patient flow issues such as level of care, discharge barriers, and estimated discharge date.<br /><br />Two clinical nurse leaders entered data during rounds using a laptop workstation or tablet. The tool was piloted for five months on two hospital medicine units, largely serving oncology and palliative care patients.<br /><br />The pilot found the navigator easy to use and not disruptive to clinical discussion. It helped ensure quality measures were addressed for all patients, allowed non-attendees to review round content, supported charge nurses with discharge planning and staffing, and made it easier to identify opportunities for retraining or resource advocacy. Over the pilot period, patient flow metrics improved, including better estimated discharge date accuracy and a reduction in average length of stay.<br /><br />Overall, the project concluded that integrating an EHR-based documentation tool into bedside interdisciplinary rounds can strengthen rounding fidelity, improve access to care plan information, and support quality improvement efforts.
