Impact of the Covid-19 Pandemic on Patients Hospitalized With Sepsis That Is Not Due to Covid-19: A Retrospective Cohort Study

This retrospective multicenter cohort study examined whether the COVID-19 pandemic affected outcomes for adults hospitalized with sepsis that was not due to COVID-19. Using electronic health record data from a U.S. hospital system, researchers compared patients admitted from March 1 to May 31 in 2019 versus the same period in 2020, excluding anyone with a positive COVID-19 status. They also assessed whether patients had COVID-19 testing, were placed in isolation, or were hospitalized during the pandemic.<br /><br />A total of 17,064 patients were identified. The 2020 cohort had a lower mean qSOFA score than the 2019 cohort, suggesting they appeared less ill on presentation. However, in-hospital mortality was similar: 19.9% in 2019 and 18.3% in 2020. This was somewhat unexpected because the lower qSOFA score in 2020 might have predicted better survival.<br /><br />When patients were divided into sub-cohorts, those in 2020 who were isolated and tested negative for COVID-19—representing persons under investigation—had a slightly higher mortality rate than 2020 patients who were not isolated and not tested, but this difference was not statistically significant (19.5% vs 17.6%). Some comparisons across cohorts showed statistically significant differences, but the key PUI comparison did not.<br /><br />The authors conclude that sepsis patients without COVID-19 hospitalized during the pandemic had mortality comparable to pre-pandemic patients despite appearing less severe clinically. They suggest that pandemic-related processes such as isolation and viral testing may have influenced inpatient outcomes, and recommend further research to better understand how healthcare system changes during COVID-19 affected non-COVID patients.
