Impact of a Multidisciplinary Team Model on Discharge Efficiency, Accuracy, and Internal Medicine Resident Perceptions on Discharge Planning

This study evaluated whether multidisciplinary rounds (MDR)—daily discharge-focused rounding with physicians, social work, and case management—improve discharge efficiency and resident education in internal medicine wards.<br /><br />MDR were implemented on 2 of 4 inpatient teams at a tertiary teaching hospital and compared with traditional teams over 8 months. Outcomes included length of stay (LOS), accuracy of estimated discharge dates (EDD), frequency of conditional discharge orders, and survey responses from residents, social workers, and case managers.<br /><br />Results showed that MDR improved discharge planning performance. Teams using MDR estimated the correct EDD more often than traditional teams (80.5% vs 76.6%, p=0.0312) and placed conditional discharge orders more frequently (61.7% vs 43.8%, p=0.0001). LOS was not significantly different, though there was a trend toward shorter stays in the MDR group (6.21 vs 6.94 days, p=0.0796).<br /><br />Resident surveys had a 65.5% response rate. More than 65% of residents reported improved ability to discharge patients safely, timely, and with better awareness after MDR implementation. More than 64% said MDR gave them more time for patient care and educational activities. Residents correctly identified social work/case management roles about 74.9% of the time, with the lowest accuracy for social work’s role in long-term care coordination and the highest for case management’s role in arranging home antibiotics.<br /><br />All surveyed social workers and case managers reported that working with residents positively affected their effectiveness.<br /><br />Overall, the study suggests that MDR improves discharge preparedness, accuracy, and resident training experience, while possibly reducing hospital stay. The authors recommend broader use of MDR, while noting limitations such as recall bias, response bias, and possible crossover between groups.
