Impact of Resident - Driven Multimodal Intervention on Telemetry Utilization and Length of Stay in Acute Care Community Hospital

This study evaluated whether a multimodal quality-improvement intervention could reduce inappropriate telemetry use and shorten hospital length of stay (LOS) in an acute care community hospital. The project was motivated by concerns that telemetry is often overused, contributing to higher costs, alarm fatigue, reduced patient mobility, and longer emergency department boarding times.<br /><br />Using a Plan-Do-Study-Act (PDSA) design, the team compared two 2-month cohorts before and after the intervention, with 150 patients in each group. The cohorts had similar acuity based on average case mix index (CMI) values of 1.67 and 1.68. The intervention included resident and hospitalist education on the 2017 American Heart Association telemetry guidelines, a structured daily telemetry checklist, and electronic health record (EHR) modifications to prompt review of telemetry necessity. Statistical analyses included Pearson correlation, simple linear regression, and Welch’s t-tests to assess relationships between telemetry days and LOS and compare pre- versus post-intervention outcomes.<br /><br />The intervention was associated with a reduction in telemetry duration and LOS. The authors conclude that a resident- and hospitalist-driven, multipronged approach can improve guideline-based telemetry utilization and may reduce patient financial burden after hospitalization. They recommend further PDSA cycles, periodic education, cost analysis, and larger multicenter studies to confirm the findings.
