Impact of Modified Barium Swallow Study on Diet Management in Hospitalized Patients With Dementia and Oropharyngeal Dysphagia

This study examined whether modified barium swallow studies (MBSS) influence diet management in hospitalized older adults with Alzheimer disease and related dementias (ADRD) who also have oropharyngeal dysphagia (OD). OD is very common in this population, and MBSS is the standard test used to diagnose and grade swallowing problems, but its effect on clinical decision-making is not well understood.<br /><br />Researchers conducted a retrospective cohort study across 11 hospitals in New York, including patients age 65 and older with ADRD and OD who had at least one bedside swallow evaluation by speech-language pathology and at least one MBSS. The main outcome was whether diet recommendations changed after MBSS. Hospital outcomes such as mortality, respiratory complications, and length of stay were also reviewed.<br /><br />The study found that MBSS led to a change in diet recommendations in 72.1% of patients, showing that the test frequently influenced clinical management. After MBSS-guided diet changes, 12.5% of patients experienced respiratory complications. Palliative care involvement was also common: 26.9% received palliative care consultation, and 20.2% chose comfort feeding.<br /><br />The authors conclude that MBSS plays an important role in shaping diet decisions for hospitalized patients with ADRD and dysphagia, but palliative care is also a key part of management. Because the study was retrospective, further research is needed to assess longer-term swallowing outcomes, readmissions, and overall survival.
