Impact of Inpatient Addiction Medicine Consultation in Hospitalized Patients

This retrospective single-center study examined whether inpatient Addiction Medicine Consultation (AMC) affected outcomes in hospitalized adults with substance use disorders (SUD). The registry included patients age 18 and older hospitalized for at least 24 hours between March 2018 and January 2022. A randomly selected convenience subset underwent chart review. Researchers compared patients who received AMC with those who did not, focusing on two outcomes: leaving against medical advice (AMA) and 30-day readmission. Both univariate and multivariable analyses were performed.<br /><br />At Cooper University Hospital, the Addiction Medicine consult service evaluated about 10% of hospitalized patients, representing roughly 50% of those with SUD. Board-certified Addiction Medicine specialists assessed patients, initiated treatment such as medications for use disorders, and referred patients to outpatient addiction care after discharge.<br /><br />The study found that patients who received AMC had similar rates of AMA compared with those who did not. However, the AMC group had lower 30-day readmission rates. The authors suggested the similar AMA rate may reflect a bias in which clinicians are more likely to consult AMC when patients are already signaling an intent to leave. The AMC group also included patients with higher rates of opioid use and injection drug use, indicating a potentially higher-risk population.<br /><br />Overall, the study suggests that inpatient Addiction Medicine consultation may reduce short-term readmissions among patients with SUD, even if it does not clearly change AMA rates. The authors concluded that these findings support the value of inpatient addiction specialty care and warrant further investigation in a larger study.
