Impact of Infectious Diseases Consultation on Staphylococcus Aureus Bacteremia: Reduced Need for Intensive Care and Inpatient Mortlaity

This observational study examined whether infectious diseases (ID) consultation improves outcomes for hospitalized patients with <em>Staphylococcus aureus</em> bacteremia (SAB) across a 30-hospital network in the Western U.S. The investigators used microbiology and electronic medical record data to identify 3,788 inpatient SAB cases and compared patients who received an ID consult (2,714; 71.6%) with those who did not (1,074; 28.4%). The main outcomes were transfer to the intensive care unit (ICU), length of hospital stay, and in-hospital mortality. The study population was ethnically diverse. Patients with an ID consult were slightly older than those without a consult, and the racial composition differed modestly between groups. The key finding was that involvement of an ID physician was associated with better inpatient outcomes. Specifically, patients seen by an ID specialist were less likely to be transferred to the ICU and had lower in-hospital mortality. The abstract concludes that ID physician involvement is beneficial in the management of SAB. Overall, this large real-world study supports the value of ID consults for hospitalized SAB patients, adding observational evidence in contrast to a recent randomized trial that did not show benefit, possibly because the control group also received stewardship support.
