Impact of Hospitalists Years of Experience on Clinical Outcomes of Inpatients

This retrospective cohort study examined whether hospitalists’ years of clinical experience influence inpatient outcomes in Korea. Using data from two tertiary centers, the researchers analyzed 4,958 patients admitted to internal medicine hospitalist wards between 2017 and 2022. Patients were grouped by their attending hospitalist’s experience: 1, 2, 3, or 4 years.<br /><br />The main outcomes were in-hospital death, 30-day and 90-day emergency room (ER) visits, and 30-day and 90-day readmissions. The analysis adjusted for age, sex, and Charlson Comorbidity Index.<br /><br />The study found that greater hospitalist experience was associated with better post-discharge outcomes. Compared with patients treated by 1-year hospitalists, those cared for by hospitalists with 2, 3, or 4 years of experience had progressively lower odds of ER visits and readmissions. For example, adjusted odds ratios for 30-day ER visits were 0.71, 0.58, and 0.55 for 2, 3, and 4 years of experience, respectively. A similar pattern was seen for 30-day readmissions, with odds ratios of 0.59, 0.50, and 0.38. These favorable trends also appeared in 90-day outcomes.<br /><br />In-hospital mortality did not differ significantly for the 2- and 3-year experience groups, but was higher in the 4-year group. The authors also noted that more experienced hospitalists tended to care for patients with lower KTAS scores, suggesting they may have managed more clinically severe cases.<br /><br />Overall, the findings suggest that increasing hospitalist experience improves care quality after discharge and may reduce pressure on emergency services. The study supports retaining experienced hospitalists to strengthen inpatient care outcomes.
