Impact of Geriatrics Consult in Hospitalized Older Adults With Trauma

This retrospective cohort study from Mayo Clinic examined whether geriatric consultation (GC) improves outcomes in older adults hospitalized for trauma at a level 1 trauma center. Patients were identified from the trauma registry and electronic health records and were age 65 or older, hospitalized between 2016 and 2022. The analysis compared 1,272 patients who received GC with 1,272 matched patients who did not, matching on age, sex, race, injury severity score (ISS), and year of admission.<br /><br />GC was typically provided to trauma-service patients aged 75 or older, or age 65 and older with frailty, while patients admitted to other services such as orthopedics, spine, or neurosurgery generally did not receive GC. Baseline characteristics were similar between groups, though the GC group had somewhat higher injury severity, more ICU stays, and more motor vehicle injuries, suggesting greater medical complexity.<br /><br />Results showed that GC was associated with significantly lower in-hospital mortality and 30-day mortality. After adjustment, GC was linked to reduced odds of hospital mortality (OR 0.31), 30-day mortality (OR 0.52), and 90-day mortality (OR 0.65). However, GC patients had a longer hospital stay by about 2 days and were more likely to be discharged to a skilled nursing facility rather than home. Readmission rates at 30 and 90 days were not significantly different between groups.<br /><br />The authors concluded that geriatric consultation may improve survival for older trauma patients, despite longer hospitalization and increased discharge to post-acute care. They suggested expanding GC services to other surgical services and emphasized the need for prospective studies to confirm benefit and reduce possible selection bias.
