Impact of Gender and Racial Disparities on Necrotizing Fasciitis Outcomes

This retrospective study examined gender and racial disparities in necrotizing fasciitis outcomes using the National Inpatient Sample from 2016–2020. The analysis included 118,774 hospitalized patients and used multivariate logistic regression to adjust for confounders.<br /><br />Overall, female patients had worse outcomes than males in several areas. Females had higher odds of invasive mechanical ventilation, blood transfusion, acute respiratory distress syndrome, and in-hospital death. They also had a slightly lower odds of some complications such as acute kidney injury, venous thromboembolism, and acute myocardial infarction.<br /><br />Marked racial differences were also observed. Compared with White patients, Asian or Pacific Islander patients had significantly higher mortality, greater need for hemodialysis, longer hospital stays, and the highest inflation-adjusted hospital charges. African American and Hispanic patients also experienced increased hemodialysis use, longer hospitalization, and higher costs. Mortality was higher among Asian or Pacific Islander and Hispanic patients. Some complication patterns varied by race, but the most consistent disparities were in renal replacement therapy, length of stay, and financial burden.<br /><br />The study highlights that necrotizing fasciitis outcomes are not equally distributed across demographic groups. Women and several racial minority groups appear to face greater clinical severity and economic burden. The authors conclude that these disparities should be addressed through targeted interventions and more equitable healthcare delivery.<br /><br />Limitations include reliance on ICD-10 administrative coding, which may cause misclassification, and the lack of laboratory values or detailed clinical severity data in the NIS database.
