Impact of Discharge Before Noon on Length of Stay

Pdf Summary
This quality improvement initiative aimed to increase discharge before noon (DBN) among hospital medicine patients from a baseline rate of 12.8% in 2021 to 15%, and to evaluate its effect on length of stay (LOS).

After approval from the hospital Quality Improvement Council, a standardized interdisciplinary DBN planning process began in July 2022. Potential next-day DBN patients were identified in advance, and case management posted patient initials and locations on the unit’s daily management board. At 2 PM each day, an EMR chat was sent to the medical team, bedside nurse, social worker, and physical therapist to confirm planned discharges and identify barriers. Any delays related to testing, medication delivery, or transportation were escalated by unit leadership. Once discharge was confirmed, physicians or advanced practice providers marked the board, and bedside nurses gave patients 24-hour notice. Teams were then encouraged to enter discharge orders before 10 AM the next morning.

The intervention focused on improving communication, accountability, and early coordination among the care team to make morning discharges more reliable. The intended operational benefit was not only to increase DBN rates, but also to reduce LOS and improve patient flow.

The document provided does not include the results or conclusions, so the impact of the intervention on DBN rates or LOS cannot be assessed from the text available.
