Impact of Adverse Event Reporting Curriculum in Graduate Medical Education

This study evaluated an educational workshop designed to improve adverse event reporting among graduate medical trainees at UC San Diego Health. Underreporting of adverse events is a major barrier to patient safety and systems improvement, and housestaff often feel unsure about reporting procedures or worry that reporting may harm professional relationships.<br /><br />The workshop included: a sample teaching case, discussion of common barriers, a breakout session for shared experiences, strategies to improve reporting, and hands-on instruction for submitting a report within UCSD Health. Participants completed pre- and post-workshop surveys using a 1–5 scale to assess changes in attitudes and competencies.<br /><br />Preliminary results from 102 residents and fellows (PGY-1 to PGY-7) suggest the intervention is effective. Sixty-seven percent had never submitted an adverse event report at UCSD. The most common barriers identified were being too busy (76%), uncertainty about whether an event was reportable (50%), not knowing how to report (43%), unclear impact of reporting (36%), forgetting (31%), fear of retaliation (22%), and lack of a reporting culture (19%). After the workshop, perceived exposure to adverse events increased from 55% to 71%, and participants reported greater comfort, knowledge, and likelihood of reporting, with statistically significant improvements (p < 0.001). <br /><br />Overall, 97% of participants found the workshop useful, and 93% felt it reduced barriers to reporting. The authors conclude that interactive, case-based education can improve housestaff awareness and confidence in adverse event reporting. Next steps include expanding the workshop to other specialties and studying its long-term impact on reporting behavior.
