Impact of Acgme Changes to Pediatric Residencies on the Pediatric Hospital Medicine Workforce

A 2024 survey of pediatric hospital medicine (PHM) leaders examined how planned 2025 ACGME residency changes may affect PHM staffing and operations. The ACGME changes are expected to increase ambulatory time and reduce resident time on inpatient pediatric services, potentially shifting more hospitalized children to hospitalist care.<br /><br />The survey, sent to leaders of 157 PHM groups, received 101 responses from programs across the U.S., including university-based and community-based settings. Results showed that many programs expect significant workforce changes: 31 anticipated fewer resident hours on PHM teams, 64 expected to increase hospitalist shifts, 68 planned to hire additional physicians, and 18 planned to hire additional advanced practice providers (APPs).<br /><br />Despite these expectations, preparation was still early. Only 26 programs had completed planning for hiring, 12 had secured funding for additional hospitalists, and 8 had begun hiring. Median staffing needs reported were 12 physician FTEs and 1.5 APP FTEs to fully staff PHM groups, with median busiest-time coverage of 5 PHM staffed teams, 3 teaching teams, and 1.5 direct-care teams.<br /><br />PHM leaders also reported concerns that hiring more staff, increasing shifts, and expanding APP roles could affect hospitalist career satisfaction. These concerns were associated with more negative perceptions of career satisfaction overall.<br /><br />The study concludes that ACGME residency changes are expected to have national impact on PHM programs, but many institutions remain in the early stages of planning. More work is needed to understand how PHM leaders are responding and how the workforce can reduce potential negative effects.
