This nationwide retrospective study compared adult hospitalizations for COVID-19 and influenza in the 2020 Nationwide Inpatient Sample using ICD-10 primary diagnosis codes. The authors examined demographic differences, comorbidities, and hospital outcomes, including mortality, discharge disposition, length of stay, mechanical ventilation, and major complications.<br /><br />Among 1,080,150 hospitalizations, 1,004,565 (93%) were for COVID-19 and 75,585 (7%) for influenza. COVID-19 hospitalizations were more common among males, Hispanics, Asians, and Native Americans. They were also associated with higher rates of obesity, diabetes with complications, pulmonary embolism, acute respiratory failure, cardiac arrest, coagulopathy, and acute ischemic stroke. In contrast, influenza hospitalizations were more common among White patients and those with congestive heart failure, atrial fibrillation/flutter, valvular disease, chronic lung disease, solid tumors, renal failure, smoking, alcohol abuse, and drug abuse.<br /><br />COVID-19 was linked to substantially worse in-hospital outcomes than influenza. Patients with COVID-19 had much higher rates of viral pneumonia, ventilator-associated pneumonia, myocarditis, ARDS, mechanical ventilation, longer hospital stays, non-home discharge, post-discharge disability, and mortality. Mortality was 11.01% for COVID-19 versus 1.30% for influenza. Influenza patients, however, had a higher odds of pneumonia overall.<br /><br />The study concludes that COVID-19 and influenza differ markedly in affected populations and clinical severity, with COVID-19 causing significantly poorer hospital and discharge outcomes. The authors suggest that further longitudinal research is needed to guide targeted interventions and improve preparedness for these respiratory infections.
