Idiopathic Pulmonary Fibrosis in Sepsis Is a Predictor of Worse Clinical Outcomes and Increase Health Care Utilization Cost

This study examined whether idiopathic pulmonary fibrosis (IPF) worsens outcomes in adults hospitalized with sepsis. Using the 2018 National Inpatient Sample, the authors identified 2,194,360 sepsis admissions, including 34,232 patients (1.56%) with IPF.<br /><br />Patients with IPF were older on average than those without IPF (72 vs 65 years) and had similar sex distribution, with some differences in race/ethnicity. After adjusting for age, sex, race, income, insurance, and comorbidities, sepsis patients with IPF had significantly worse clinical outcomes.<br /><br />Compared with non-IPF patients, the IPF group had:<br />- Higher in-hospital mortality: 13.8% vs 8.9% (adjusted OR 1.47, 95% CI 1.36–1.59)<br />- Greater need for invasive mechanical ventilation: 11.8% vs 8.6% (adjusted OR 1.30, 95% CI 1.20–1.42)<br />- Higher mean hospital charges by about $16,082<br /><br />There was no significant difference in hospital length of stay.<br /><br />The authors conclude that IPF is a predictor of poorer outcomes and increased healthcare costs among patients admitted with sepsis. They suggest that early prognostic discussions with patients and families may help guide goals of care, especially given the severe illness burden in this population.
