This abstract describes the development of an Inpatient Transition Consult (ITC) for hospitalized adolescents and young adults with childhood-onset chronic disease at Duke University Hospital. The authors note that while national guidelines support structured pediatric-to-adult transition in ambulatory care, there is little guidance for hospital-based transition models. Because hospitalization may be a key opportunity to identify patients who are eligible for adult care or overdue for transfer, the team created an ITC staffed by medicine-pediatrics physicians and a licensed clinical social worker.<br /><br />The ITC includes six core components: a comprehensive medical summary, review of social determinants of health, transition readiness assessment and counseling, advance care planning, care coordination with outpatient teams, and assessment of readiness for future admissions to adult wards.<br /><br />To evaluate the program, the authors reviewed charts of patients who received an ITC from October 2023 to July 2024. During this period, 30 ITCs were completed for 28 distinct patients. The preliminary review aimed to assess progress toward inpatient health care transition and identify themes or outcomes plausibly attributable to the consult, including patient and caregiver satisfaction, length of stay, and readmission rate.<br /><br />The project suggests that an inpatient transition consult may help improve transition preparedness for hospitalized youth with special health care needs and support more timely movement to adult-oriented care. The authors plan to expand the model with a dedicated ITC consult service starting in 2025 and further study its impact on transition outcomes, satisfaction, and hospital utilization.
