This retrospective single-center study examined whether prolonged nothing-by-mouth (NPO) times increase hypoglycemia risk in diabetic patients undergoing GI procedures. The authors reviewed 69 adults with diabetes who underwent EGD, ERCP, or colonoscopy at Wellstar Kennestone Regional Medical Center between August 1, 2023, and August 1, 2024.<br /><br />The study was motivated by guidelines recommending short fasting periods before sedation, yet prior research suggested actual NPO times for GI procedures often exceed 12 hours. Since extended fasting may contribute to hypovolemia or hypoglycemia, the investigators measured NPO duration and documented hypoglycemic events.<br /><br />Among the 69 patients, 8 experienced hypoglycemia. The cohort had a mean age of 65.4 years; 60.9% were male and 50.7% were white. The average NPO time was 12.2 hours, indicating fasting often exceeded recommended durations. However, prolonged NPO time was not significantly associated with hypoglycemic events (p = 0.248). Linear regression also showed only a weak relationship between HbA1c levels and NPO length.<br /><br />The authors conclude that, in this sample, longer NPO times were not linked to more hypoglycemia in diabetic patients undergoing GI procedures, despite NPO durations being longer than recommended. They note that because hypoglycemia can have serious consequences, larger studies are needed to better assess risk and strengthen statistical analysis.
