How Health-Related Social Needs Impact Readmissions in Frequently Hospitalized Patients

The authors suggest that these forms of insecurity may make readmission reduction especially difficult because they are often tied to broader financial challenges that patients cannot easily solve on their own.<br /><br />The main conclusion is that future interventions for frequently hospitalized patients may be more effective if they specifically target food and transportation insecurity, since these needs appear to be linked with higher readmission risk.
