How Do Inr and Anticoagulants Impact the Rate of Major Bleeding in Hospital Medicine Patients Undergoing Bedside Procedures?
Back to course
Pdf Summary
The document presents a retrospective chart review examining whether elevated INR levels and anticoagulant use increase major bleeding risk during bedside procedures on a hospital medicine service. The study reviewed procedures performed between February 2020 and August 2021, comparing 275 procedures with INR values above 2 to 272 procedures with INR values of 2 or below. Many patients were receiving either prophylactic or therapeutic anticoagulation.

The primary outcome was major bleeding, defined as either a hemoglobin drop of more than 2 g/dL or the need for 2 or more units of packed red blood cells. Across the procedures, there were 7 major bleeding events total: 2 in the elevated INR group and 5 in the lower INR group. No bleeding events were linked to anticoagulant use, and there were no deaths. Most patients undergoing these procedures had cirrhosis or alcoholic hepatitis.

The findings suggest that bedside procedures performed in hospital medicine patients do not show a significant increase in major bleeding even when INR is elevated or when patients are receiving anticoagulation. The conclusion is that these procedures appear to be relatively safe without requiring routine correction of INR beforehand.
