Hospitals That Serve Socially Deprived Populations Experience Higher Readmission Rates for These Common Clinical Conditions

This study examined whether neighborhood social determinants of health (SDOH), measured by the Social Deprivation Index (SDI), are associated with 30-day hospital readmission rates for pneumonia, heart failure (HF), and acute myocardial infarction (AMI). The authors used national data from CMS hospital readmission records (2005–2019), U.S. Census demographic and economic data, Dartmouth Atlas hospital catchment area mappings, and ZIP-code-level SDI estimates. These datasets were merged to assign each hospital an SDI percentile based on the average deprivation of ZIP codes in its service area.<br /><br />Hospitals serving communities in the highest SDI quartile had higher readmission rates than those in the lowest quartile. In unadjusted analyses, readmissions were higher by 1.12% for HF, 0.70% for AMI, and 0.65% for pneumonia. After adjusting for potential confounders, the association remained significant: compared with the lowest SDI quartile, the highest quartile had readmission rates higher by 0.88% for HF, 0.40% for AMI, and 0.55% for pneumonia.<br /><br />The findings suggest that community-level social deprivation is linked to worse hospital readmission outcomes across these common conditions. The study highlights the importance of considering neighborhood SDOH in evaluating hospital performance and designing strategies to reduce readmissions. The authors conclude that interventions addressing social disparities are needed to improve patient outcomes nationwide.
